Form 5
All practicing PAs with a delegating physician MUST fill out the NEW Form 5. It does not matter if you are writing for Sched II’s (inpatient or outpatient) or not.
Due to the passage of HB2584 Form 5 has been updated. This form was updated August 29, 2025, and any Form 5 submitted prior to August 29, 2025 is INVALID, and the PA and delegating physician are non-compliant with the updated law. The NEW Form 5 is also your Practice Agreement with your delegating physician(s). All practicing PAs must have a Form 5 for each delegating physician they work with.
All PAs prescribing Sched IIs MUST have a new Form 5 on file with OMB complete with ALL practice addresses. Pharmacies check this information. PAs may check the OMB website to confirm they are on the list of PAs authorized to prescribe Sched II drugs at specified locations. PAs CAN NOT prescribe Sched IIs until their Form 5 has been UPDATED with OMB and the PA has been added to the list of PAs authorized to prescribe Sched II drugs.

Form 6
All PAs that wish to practice independently MUST fill out a Form 6. Independent Practice is NOT implied.
Form 6 must be filled out and the PA listed as Independent Practice on the OMB website prior to practicing independently.
You may check the OMB website to see the list of PAs authorized for Independent Practice. A PA that has filled out a Form 6 CAN NOT practice independently until they have been added to the list of PAs authorized for Independent Practice.
