Dr. Neil Agrawal is a non-invasive cardiologist at the Saint Francis Heart and Vascular Institute in Tulsa, Oklahoma, where he serves as Director of the Coronary CTA Program and founder of the Preventive and Metabolic Cardiology Clinic. He is board certified in cardiology and cardiovascular computed tomography with advanced expertise in coronary CTA, preventive cardiology, structural and congenital cardiac CT imaging, echocardiography, and nuclear cardiology.
Dr. Agrawal has led multiple cardiovascular imaging and prevention initiatives, including implementation of AI-based coronary plaque analysis and CT-derived fractional flow reserve programs. He serves on the Society of Cardiovascular Computed Tomography (SCCT) Health Policy and Private Payer Task Force, participates in EPIC’s Cardiac CTA Brain Trust, and is a member of the American College of Cardiology Cardiovascular Imaging Section Leadership Council.
His interests focus on precision cardiovascular prevention, advanced cardiac imaging, coronary plaque characterization, and expanding access to preventive cardiovascular care in both community and rural settings.

- Discuss the evolution of cardiovascular risk assessment from Framingham risk factors and traditional risk calculators to modern biomarker- and imaging-guided prevention strategies, with emerging roles for genetic risk assessment.
- Understand how biomarkers, coronary artery calcium (CAC), and plaque characterization on coronary CTA can improve cardiovascular risk assessment beyond traditional risk factors alone.
- Apply biomarker and imaging findings to individualized preventive treatment strategies, including lipid-lowering and cardiometabolic therapies, through shared decision-making frameworks using real-world clinical cases.
